The Square Dental Clinic Privacy Policy

The Square Dental Clinic (hereinafter referred to as the “Clinic”) places great importance on the protection of your personal information and complies with the Personal Information Protection Act of the Republic of Korea. 1. Personal Information Collected and Method of Collection

The Clinic collects only the minimum personal information necessary for the use of its services upon membership registration. Membership registration includes required and optional items; you may use the services without restriction even if you do not provide optional items such as consent to receive emails.

    - Items collected: Name, address, contact information, email, date of birth, gender, etc.
    ※ Under the Medical Service Act, the Clinic is required to retain unique identification information and medical information (separate consent is not required).

2. Purpose of Collection and Use of Personal Information

The Clinic uses the collected personal information for the following purposes. All information provided by users will not be used for any purpose other than those stated below, and prior consent will be obtained if the purpose of use changes.

    - Provision of medical services for diagnosis and treatment, and administrative services such as billing, payment, and refunds

The Clinic destroys your personal information without delay once the purpose for which it was collected or provided has been achieved.

    - Until the member cancels their membership or is removed from membership. However, even if the purpose of collection or provision has been achieved, the Clinic may retain your personal information if it is necessary to preserve it under the provisions of laws such as the Commercial Act.

    - Records of identity verification: 6 months (Act on Promotion of Information and Communications Network Utilization and Information Protection, etc.)
    - Records of visits: 3 months (Protection of Communications Secrets Act)

4. Procedures and Methods for Destruction of Personal Information

The Clinic destroys personal information immediately after the “purpose of collection and use of personal information” has been achieved. The procedures and methods of destruction are as follows.

    Destruction Procedure

    - Information entered by users for membership registration, etc. is destroyed immediately in accordance with the destruction method once the purpose has been achieved.

    - Personal information stored in electronic file format is deleted using technical methods that make the records unrecoverable.
    - Personal information printed on paper is destroyed by shredding or incineration.

5. Rights of Users and Legal Representatives and How to Exercise Them

Membership registration for children under the age of 14 (hereinafter referred to as “Children”) is carried out through a separate form written in plain language that Children can easily understand, and the consent of a legal representative is always obtained when collecting personal information. To obtain the consent of the legal representative, the Clinic collects the minimum information necessary from Children, such as the name and contact information of the legal representative, and obtains the legal representative's consent in accordance with the methods set forth in this Privacy Policy. The legal representative of a Child may request access to, correction of, or deletion of the Child's personal information. To access, correct, or delete a Child's personal information, the legal representative may click “Edit Member Information” and, after completing the legal representative verification procedure, directly access, correct, or delete the Child's personal information, or contact the Personal Information Manager in writing, by telephone, or by fax, and the Clinic will take the necessary measures. The Clinic does not provide or share information about Children with third parties, and if a legal representative requests correction of an error in personal information collected from a Child, the Clinic will not use or provide such personal information until the error has been corrected.

  • Personal information that is required to be retained by law cannot be modified or deleted during the retention period, even upon request.

6. Withdrawal of Consent / Membership Cancellation

You may withdraw at any time the consent you gave to the collection, use, and provision of personal information upon membership registration. To cancel your membership, you may click “Cancel Membership” on My Page of the Clinic's website and cancel your membership directly after completing the identity verification procedure, or contact the Personal Information Manager in writing, by telephone, or by fax, and the Clinic will take the necessary measures, such as destroying your personal information without delay.

7. Installation, Operation, and Refusal of Devices that Automatically Collect Personal Information

The Clinic uses “cookies” that store and retrieve your information from time to time. A cookie is a very small text file sent to your browser by the server used to operate the Clinic's website, and it is stored on your computer's hard disk. The Clinic uses cookies for the following purposes. You have the choice of whether to accept cookies. Accordingly, by setting the options in your web browser, you may allow all cookies, be asked for confirmation each time a cookie is stored, or refuse to store any cookies. If you refuse the installation of cookies, it may be difficult to provide some services.

9. Measures to Ensure the Security of Personal Information

As a technical measure to protect users' personal information, the Clinic has put in place various security devices. All information sent by users is safely stored and managed in a security system protected by a firewall. In addition, as an administrative measure to protect users' personal information, the Clinic has established the procedures necessary for access to and management of users' personal information, minimizes the number of personnel who handle users' personal information, and provides continuous security training. The Clinic also designates the users of systems that process personal information, assigns them passwords, and renews these passwords regularly.

10. Notification of Policy Changes

This Privacy Policy was established on April 1, 2012. If there are any additions, deletions, or modifications due to changes in laws, policies, or security technologies, the Clinic will announce the reasons for and details of the changes on its website at least 7 days before the revised Privacy Policy takes effect.
